SAMH

If a customer has Substance Abuse & Mental Health issues, they may be committed to a treatment program. The SAMH page keeps track of their treatment program with relevant dates, counselor name, hours of treatment, etc.

Steps	Action
1	Click on the Alternative Plan tab on Case Detail.
2	Click the 'Add' button on the SAMH tab.
3	Enter the following information: Total Assigned Countable Hours/Week Number of Hours in Treatment/Week Date SAMH Treatment Verification Form Documented Current Treatment Hours from Form CF-ES 2299 SAMH Counselor Name SAMH Counselor Phone Months Earned and Credited (as verified by PAS) Date Months Earned and Credited was Entered Begin Date of Treatment Anticipated End Date of Treatment Status of SAMH Treatment
4	Click the 'Save' button.
