A deferral may be added for a customer if they have a medical reason for why they are unable to work. To complete this page, you need to supply physician information that verifies their medical excuse.

Respond to the question, 'Is customer enrolled on an SAMH Outpatient Treatment Program?' |

Enter a Deferral Begin Date. |

Enter an Anticipated Deferral End Date. |

Enter an Initial Follow-up Date. |
